Brookhaven Nursing and Rehabilitation

Dynasty Healthcare, LLC owns and operates Brookhaven Nursing and Rehabiliation, a rehabilitation and long-term care community in North Carrollton, located directly across the street from Baylor Carrollton.

Brookhaven Nursing and Rehabilitation Center offers physical, occupational and speech therapy to meet the needs of residents with orthopedic, neurological and other diagnoses. In addition to physical strength, range of motion, balance and activities of daily living, our team of therapists will address cognitive, memory, social and emotional needs that derive from injury or illness. We provide rehabilitation services through a comprehensive plan of care established by our medical team that maximizes each resident's potential for greater independence.

Our Long Term Care residents live with us when they require some degree of nursing care and observation. Twenty-four hour nursing care is the step up from Assisted Living. Our certified nurse aides provide assistance with all Activities of Daily Living (ADLs). The Long Term Care experience is focused on community and includes a full schedule of optional activities run by two full-time Activities personnel.
